Oxford cut its New York City small-group lineup from 41 plans to 26 for 2027, dropping every bronze and HSA plan it sold in 2026. For most New York teams, I'd start a new plan on December 1, 2026. Apply now. If you can't meet participation minimums, apply between November 15 and December 15 for a January 1 start.

- **Oxford NYC plans:** 41 in late 2026, 26 in 2027
- **Oxford plans without a deductible:** Platinum, from $1,762.69/mo
- **Prescience:** Effective $0 deductible
- **Start date:** December 1, 2026

## What Oxford cut for 2027

I compared Oxford's New York City small-group plans for a December 1, 2026 start with its plans for January 1, 2027. The December list has **41 plans**. The January list has **26 plans**. Both counts leave out Healthy NY, the state's separate program for qualifying small businesses. Oxford's filings with the New York Department of Financial Services (DFS) show the same cut.

**Oxford NYC small-group plans by metal level, excluding Healthy NY**

| Plan type | December 1, 2026 start | January 1, 2027 start |
| --- | --- | --- |
| Bronze | 5 | 0 |
| Silver | 15 | 8 |
| Gold | 14 | 11 |
| Platinum | 7 | 7 |
| HSA-eligible (any metal) | 15 | 3, all gold |
| Networks | Freedom, Liberty, Metro | Freedom, Liberty |
| Total | 41 | 26 |

Oxford removed all 5 bronze plans, 8 of its 15 silver plans and all 15 of its HSA-eligible plans, then added 4 new plans: 1 silver and 3 gold HSA plans. It also dropped its Metro network. The low-premium end of the menu is gone. A New York company shopping for January 2027 coverage is choosing between a high-deductible silver plan and a much more expensive gold or platinum plan.

## What skipping the deductible costs in New York in 2027

Most of the founders I talk to don't want a cheap plan or an expensive plan. They want a plan their team can actually use: the plan pays from the first visit, and costs people know before a visit. In Oxford's 2027 New York City lineup, the plans without a deductible that also have a low out-of-pocket maximum are platinum. The cheapest is **$1,762.69 a month** for one person.

**Oxford NYC 2027 plans for a single employee, January 1, 2027 start, next to Prescience**

| | Oxford silver | Oxford gold | Oxford platinum | Prescience |
| --- | --- | --- | --- | --- |
| Plan | Liberty 30/60/4500/50 EPO | Freedom 15/35/1750/90 EPO | Freedom 20/40/100 EPO | Prescience group plan |
| Monthly premium, one person | $1,206.76 | $1,519.20 | $1,762.69 | Set in your quote, usually 10–20% below comparable fully insured coverage |
| Deductible | $4,500 | $1,750 | $0 | Effective $0 |
| Out-of-pocket maximum | $12,000 | $9,000 | $3,250 | Low, capped |
| What employees see | Full cost until the deductible | Full cost until the deductible | Copays | Simple copays shown before a visit |
| Help finding care | Carrier directory | Carrier directory | Carrier directory | 24/7 physician-led care navigation |

The silver plan looks affordable until someone gets sick. Then an employee pays the first $4,500 of most care themselves, and up to $12,000 in a bad year. Platinum fixes that, at a premium about 46% higher than silver. That gap is the trade most New York employers face in 2027: a usable plan or an affordable one.

**Ten employees, single coverage, one year**

Illustrative, using Oxford's January 1, 2027 NYC rates. Ten employees on single coverage, the company paying the full premium. The Prescience line applies our published range to the platinum premium to show the scale. It is not a quote: your cost depends on your team and is set in your quote.

- Oxford silver, $4,500 deductible: 10 × $1,206.76 × 12 = $144,811.20
- Oxford platinum, deductible $0: 10 × $1,762.69 × 12 = $211,522.80
- 10–20% below the platinum premium: $169,218.24 to $190,370.52
- Difference from platinum: $21,152.28 to $42,304.56 a year

At platinum prices, dropping the deductible costs this team $66,711.60 a year more than silver. Prescience's effective $0 deductible is meant to close most of that gap, so the team gets the plan it can use without paying platinum premiums.

> **Prescience Diamond: A company health plan with an effective $0 deductible.** For New York teams facing Oxford's smaller 2027 menu: an effective $0 deductible from the first visit, and one monthly cost set before the plan starts, usually 10–20% below comparable fully insured coverage, with no admin fees.
> - **An effective $0 deductible** and simple copays shown before a visit
> - **A broad national PPO network,** plus providers we contract with directly
> - **24/7 physician-led care navigation** that books visits and explains costs
> - **Usually 10–20% below comparable fully insured coverage,** fixed in your quote, with no admin fees

## Why I'd start December 1, 2026

A small-group plan's price is set by its start date and holds until the group's renewal, normally 12 months later. That makes December 1, 2026 the last start date before the 2027 menu takes over. A company that starts December 1 has its first renewal on December 1, 2027, not January 1, 2027.

- **You get a full year before 2027 pricing reaches you.** Your plan and monthly cost are set now and hold until December 2027.
- **You keep plans that won't be sold to new groups after December.** That includes Oxford's bronze and silver HSA plans and the Metro network.
- **You get coverage in December.** If your team has no plan today, the December month is coverage you want anyway.
- **You don't lose a month of deductible.** Oxford's small-group deductibles run on the plan year, and Prescience gives an effective $0 deductible from the first visit.

One honest caveat. Oxford uses quarterly rates in New York, and its fourth-quarter 2026 prices are higher than its January 2027 prices for the plans it keeps. If you are buying one of those 22 Oxford plans directly, January 1 is 2% to 6% cheaper on the same plan. The case for December is the plans that disappear, and for us the effective $0 deductible. Ask for quotes on both dates and compare them on the same census.

> **Can't meet the minimums? Apply November 15 to December 15:** Insurers can turn down a small group that doesn't meet their minimum participation or employer contribution rules. Federal rules make one exception: from **November 15 to December 15** each year, an insurer can't turn a group down for missing those minimums. If some employees plan to waive, or the company can't pay the usual share of premiums, apply in this window for a January 1 start.

Mind the start date. Coverage bought in this window starts January 1, so it gets the 2027 menu, not the plans being cut. A December 1 start is for groups that meet the minimums: apply now, before the December 1 deadline. If you aren't sure whether you meet them, ask your broker or the insurer before you apply.

**Quote a December 1 start before the window closes**

Request December 1 and January 1 quotes on the same census and contribution, and put each plan's deductible next to its premium. Groups that can't meet participation minimums can apply November 15 to December 15 for January 1.

## How to start a plan on December 1

1. **Apply now** Insurers need the application, census and first payment before the start date. Work back from the December 1 deadline. If you can't meet participation or contribution minimums, plan instead to apply between November 15 and December 15 for January 1.
2. **Price both dates on one census** Request December 1 and January 1 quotes for the same employees, tiers and contribution, including a Prescience quote. Compare family and couple rates, not only single rates.
3. **Compare deductibles, not just premiums** Put each plan's deductible and out-of-pocket maximum next to its premium, as in the table above. A low premium with a $4,500 deductible is not a cheap plan for the employee who needs care.
4. **Check doctors on the exact network** Have employees check their doctors and prescriptions against the plan's network name before you choose.
5. **Tell employees what changes** Explain the start date, what they pay per paycheck and how to get care in December. If the plan is HSA-eligible, explain the HSA timing below before anyone contributes.

## HSAs and a December start

Oxford has no silver HSA-eligible plan for 2027. Its six silver HSA plans end for new groups after 2026, and its only 2027 HSA plans are three gold plans, starting at $1,457.35 a month for one person. That is 33.9% more than the cheapest Oxford HSA plan for a December 2026 start, a $6,500-deductible bronze plan at $1,087.99.

If an employee is covered by an HSA-eligible plan on December 1, 2026, the IRS last-month rule treats them as eligible for all of 2026. They can contribute up to the full 2026 limit of $4,400 for self-only coverage or $8,750 for family coverage. The catch is the testing period: they must stay HSA-eligible through December 31, 2027. If the group renews on December 1, 2027 into a plan that isn't HSA-eligible, anyone who used the last-month rule owes income tax and a 10% additional tax on the extra contribution. Contributing only the December share, one-twelfth of the annual limit, avoids that.

## The 35% rate increase you may have heard about

Oxford asked DFS for a **30.6%** average small-group increase for 2027. DFS approved **6.3%**. Across all New York small-group insurers, requests averaged 23.7% and approvals 8.0%. A broker summary published before the decision listed Oxford's request at 34.5%, which is probably where the 35% figure comes from.

The approved average is not what every company pays. If your current plan is one of the 19 Oxford cut, your renewal moves you to a replacement plan, and the replacement's price, deductible and network can all differ. That, not the market rate, is where the big renewal increases come from. Ask your broker or Oxford in writing which plan you'll be moved to, and price the alternatives before you accept it.

## Mistakes to avoid

- **Waiting too long.** A December 1 start needs your paperwork in before the December 1 deadline. After that, the earliest start is January 1.
- **Comparing premiums alone.** Oxford's cheapest 2027 plan has a $4,500 deductible. Put the deductible next to the premium before deciding a plan is cheap.
- **Comparing a 2027 quote with a 2026 rate sheet.** Oxford's price depends on the quarter you start. Compare quotes for the dates you would actually start.
- **Assuming a renewal keeps your plan.** If Oxford cut your plan, you'll be moved to a different one. Ask which.
- **Forgetting Long Island and upstate.** These figures are for New York City. Oxford also sells on Long Island and in the Mid-Hudson region, where the plan list and prices differ.

## Frequently asked questions

**Should a New York company start health insurance December 1, 2026 or January 1, 2027?**

For most teams, December 1. It is the last start date before Oxford's smaller 2027 menu, the price holds until December 2027, and the December month is coverage. If you are buying one of the Oxford plans that continue into 2027, January is 2% to 6% cheaper on that plan.

**Is Oxford raising small-group rates 35% in 2027?**

No. Oxford requested a 30.6% average increase for 2027, and DFS approved 6.3%. A renewal much higher than that usually means your plan was cut and replaced.

**How many small-group plans does Oxford offer in New York City for 2027?**

26, excluding Healthy NY, down from 41 for a December 2026 start. Oxford dropped every bronze plan, 8 of its 15 silver plans, all 15 of its HSA plans and its Metro network, and added 1 silver and 3 gold HSA plans.

**Which Oxford 2027 plans don't have a deductible?**

In New York City, its platinum plans, from $1,762.69 a month for one person, plus a few silver and gold plans without a deductible but with out-of-pocket maximums of $10,000 to $12,000. Prescience gives an effective $0 deductible with a low out-of-pocket maximum.

**Does Oxford have a silver HSA-eligible plan in 2027?**

Not in New York City. Its six silver HSA plans end for new groups after 2026. The only 2027 HSA plans are three gold plans, starting at $1,457.35 a month for one person.

**Can we get coverage if we don't meet the participation minimum?**

Yes, if you apply between November 15 and December 15, when federal rules don't let insurers turn a small group down for missing participation or contribution minimums. Coverage then starts January 1, on the 2027 menu.

**Do we lose a month of deductible by starting December 1?**

No. Oxford's small-group deductibles run on the plan year, so a December 1 start runs the deductible from December 1 to November 30. With Prescience's effective $0 deductible, there is nothing to reset.
